General Liability Waiver

As the legal owner/agent of the above-mentioned pet(s), having carefully read and fully understand this agreement, do hereby waive and release YVAS, directors, trainers, employees, and representatives, and their dog(s) from any and all liability of any nature. This includes any injury, death, sickness, or damage my pet may suffer during or after any training program. I also agree to indemnify and hold harmless YVAS, directors, trainers, employees, and representatives, and their dog(s) from any and all claims due to damage the pet may cause to any family members of any third parties during or after training. The owner specifically acknowledges that they recognize the risk of taking a group class with other people and their dogs. This risk is entered into with the owner 100% responsible for their dog’s behavior at all times in the class environment.

Vaccinations Waiver

Owner certifies that his/her dog has been in Owner’s possession and has resided within Owner’s home for at least two weeks.

Owner hereby certifies that his/her dog entered into training class has been started on a course of vaccines (i.e. Bordetella, Distemper, Rabies) or has received a full course of vaccines recommended for their age OR can show results from titer tests confirming antibodies.

Owner agrees to provide YVAS with an updated list of current vaccinations given to your dog(s) signed by your veterinarian, or titer results; otherwise, the dog is not allowed in class.

Vaccinations and medical history can be obtained by YVAS and its staff from your veterinarian at any point in time.

Owner further certifies that his/her dog has been checked for, and is free from, any worm infestations before being brought to class. Any dogs showing any signs of infections, viruses, or worms will immediately be excused from class. YVAS will require a veterinarian’s note verifying the health status of the dog before allowing the dog to return to its training programs at a later date; to be mutually agreed upon by YVAS and Owner.

Owner is aware that their dog being around other dogs gives them a higher chance of contracting parasites and illness. YVAS will do its best to keep the environment as clean as possible; however, YVAS does not know where owners take their dogs when they are not in class.

I consent to the above conditions and understand YVAS is not responsible for any injury or loss.

Termination of Training

The owner of the dog(s) may have the animal withdrawn from training at any time, however, once training has begun; all fees are non-refundable.

Miscellaneous

YVAS cannot make any guarantee regarding the outcome of any training/counseling program. The owner assumes full responsibility for the dog’s behavior during and after the training/counseling program.

Owner will be responsible for purchasing all the necessary equipment that the trainer recommends for training.

Owner gives permission for photos of their dog to be taken by YVAS for use in advertising and promotional materials and media. YVAS agrees not to sell or distribute these photos to any third party.

In the event either party deems it necessary to employ legal counsel to protect its rights under this agreement, the prevailing party agrees to pay all expenses including, but not limited to costs and reasonable attorney’s fees.
